Freeform
Form solution for building complex questionnaires with branching questions
Freeform is a self-hosted alternative to Google Forms meant for (but not limited to) social sciences researchers who need to create, manage, and apply survey campaigns. It provides tools for building complex questionnaires with branching questions that adapt to participant responses, rich data types, and repeatable fields for collecting variable-length groups such as family member information or itemized expenses. It includes an admin interface for managing campaigns and downloading collected results. It allows for usage in the field with limited internet access, form styling and offers basic CMS capabilities. With an API based on PostgREST, it is easy to deploy in almost any infrastructure.
